Our experienced team help participants move from hospitals to medium or long-term housing and offer 24/7 support for those with complex health or mental health needs.

We carefully assess each participant and create personalised support plans and training to fit each person's needs. Our adaptable and responsive approach guarantees prompt, professional support.
Transition back to community
We manage the process of transitioning you back into the community, in collaboration with your entire support network.
Allied Health coordination
We coordinate with our Allied Health partners to ensure your home or supported living environment is safe and ready for you.
Connect with accommodation
Our team connects you with suitable Medium Term Accommodation (MTA) options for a safe discharge and we support further planning to transition you to a long-term home.
Provide 24/7 Support
We provide highly trained support workers to deliver your 24/7 Supported Independent Living supports.
Personalise Roster of Care
We help develop a person-centred support plan and Roster of Care that suits your individual needs and goals.
Help reduce hospital re-admission
We ensure there is complete oversight of your clinical care needs to reduce risk of hospital re-admission.
Supported independent Living (SIL)
Supported Independent Living (SIL) is a NDIS support so you can live in accommodation with other people, or on your own, and receive support and services to live life on your terms.
Our team provide the in-home supports needed for you with our ‘Supported Independent Living’ (SIL) service. The kind of SIL support you receive will be different depending on your individual needs, but can include:

Daily Living
- Personal care including showering, dressing, getting ready for the day
- Support with everyday household tasks such as grocery shopping, meal preparation and household chores
- Assisting to look after your health and fitness

Independence
- Help with transport to activities or learning to use public transport
- Support to connect with friends, access the community, clubs & activities or starting a new hobby
- Moving in and learning to live in your own home

Health & wellbeing
- Help take your medications
- Access to therapy & clinical services, like speech pathology or physiotherapy
- Post-hospital care & recovery support
- Complex medical conditions following review of your clinical needs
