Are you eligible?
You may qualify for STRC if:
- You are at least 65 years old.
- You are not currently on a Home Care Package.
- You have not been hospitalised in the last three months.
- You have not received Transition Care in the last six months.
- You are not living in an aged care facility
To access STRC, an Aged Care Assessment Team (ACAT) will assess your needs to determine your eligibility for support to live independently. We can assist you in arranging this assessment through My Aged Care.

How to apply
The process is pretty simple, if you're eligible:- Contact My Aged Care by visiting My Aged Care or call 1800 200 422 to start the application process.
- Then receive your face-to-face assessment to determine your needs.
